Pharmacy fee
Please read AAPA Member Code of Conduct agreement of commitment. Fill, sign and mail form with your filled application.

The Application must be filled out completely and mailed with fees. Pharmacy: $750. Make check payable to Arab American Pharmacist Association. If you choose to pay online you must still mail the application into our office. Once we received the signed application and payment we will begin the process of approving your application.

If your application is approved your check will be deposited and fees will be prorated and you will be refunded the amount based on the time of approval. If your application is rejected or disapproved, your check will be returned by mail.
